RESUMO

Lung cancer remains the leading cause of cancer mortality worldwide. Small cell lung cancer (SCLC) accounts for 10-15% of cases and has an overall 5-years survival rate of only 15%. Neuron-specific enolase (NSE) has been identified as a useful biomarker for early SCLC diagnosis and therapeutic monitoring. This work reports an electrochemical immunosensing platform based on a graphene-graphitic carbon nitride (g-C3N4) nanocomposite for ultrasensitive NSE detection. The g-C3N4 nanosheets and graphene nanosheets were synthesized via liquid exfoliation and integrated through self-assembly to form the nanocomposite. This nanocomposite was used to modify screen-printed carbon electrodes followed by covalent immobilization of anti-NSE antibodies. The unique properties of the graphene-g-C3N4 composite facilitated efficient antibody loading while also enhancing electron transfer efficiency and electrochemical response. Systematic optimization of experimental parameters was performed. The immunosensor exhibited a wide linear detection range of 10 pg/mL to 100 ng/mL and low limit of detection of 3 pg/mL for NSE along with excellent selectivity against interferences. Real serum matrix analysis validated the applicability of the developed platform for sensitive and accurate NSE quantifica-tion at clinically relevant levels. This novel graphene-g-C3N4 nanocomposite based electro-chemical immunoassay demonstrates great promise for early diagnosis of SCLC.

ObjectiveTo investigate the different effects of Yunvjian with or without Achyranthis Bidentatae Radix on glucose and lipid metabolism and inflammatory response in diabetic rats with the syndrome of Yin deficiency and internal heat. MethodThe rat model of diabetes due to Yin deficiency and internal heat was established by feeding with a high-sugar and high-fat diet and injection of thyroxine and streptozotocin. The successfully modeled rats were randomized into model control, Yunvjian without Achyranthis Bidentatae Radix (11.8 g·kg-1), Yunvjian with Achyranthis Bidentatae Radix (12.8 g·kg-1), and Achyranthis Bidentatae Radix (1.0 g·kg-1) groups (n=10), and another 10 rats were taken as the normal control group. Each group was administrated with corresponding drugs or saline by gavage for 28 days. The fasting blood glucose (FBG), fasting insulin (FINS), total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) in rats were measured. Enzyme-linked immunosorbent assay was employed to determine the levels of cyclic adenosine monophosphate (cAMP), cyclic guanosine monophosphate (cGMP), triiodothyronine (T3), thyroxine (T4), interleukin (IL)-1β, IL-6, tumor necrosis factor (TNF)-α, and C-reactive protein (CRP) in the serum. The histopathological changes of the liver were observed. The expression of lipoxygenase-2 (COX-2) was detected by immunofluorescence. The mRNA levels of nuclear transcription factors-κB (NF-κB), monocyte chemotactic protein-1 (MCP-1), and intercellular adhesion molecule-1 (ICAM-1) were determined by real-time polymerase chain reaction (Real-time PCR).Western blot was employed to determine the protein levels of NF-κB in hibitory protein(IκB) kinase β (IKKβ), IκBα, and phosphorylated IκBα (p-IκBα) in the liver and the protein levels of NF-κB in the cytoplasm and nucleus. ResultCompared with the normal group, the model group showed elevated levels of FBG, FINS, insulin resistance index, TC, TG, LDL-C, cAMP, T3, T4, IL-1β, IL-6, TNF-α, and CRP, up-regulated mRNA levels of NF-κB, MCP-1, and ICAM-1, and up-regulated protein levels of COX-2, p-IκBα, and nuclear NF-κB (P<0.01). Compared with the model group, Yunvjian without Achyranthis Bidentatae Radix lowered the levels of FBG, FINS, insulin resistance index, TC, TG, LDL-C, cAMP, T3, T4, IL-1β, IL-6, TNF-α, and CRP, down-regulated the mRNA levels of NF-κB, MCP-1, and ICAM-1, and down-regulated the protein levels of COX-2, p-IκBα and nuclear NF-κB (P<0.05, P<0.01). Compared with the Yunvjian without Achyranthis Bidentatae Radix, Yunvjian with Achyranthis Bidentatae Radix showed lowered levels of FBG, FINS, insulin resistance index, and inflammatory cytokines, down-regulated mRNA levels of NF-κB, MCP-1, and ICAM-1, and down-regulated protein levels of p-IκBα and nuclear NF-κB (P<0.05, P<0.01). ConclusionAchyranthis Bidentatae Radix can enhance the performance of Yunvjian in reducing blood glucose and inhibiting inflammation in diabetic rats with the syndrome of yin deficiency and internal heat by down-regulating the IKK/IκB/NF-κB signaling pathway.

In order to evaluate the safety and effectiveness of endoscopic hemorrhoids treatment, a retrospective analysis was conducted on data of 166 patients with grade I to Ⅲ hemorrhoids who underwent endoscopic treatment in the First Affiliated Hospital of University of Science and Technology of China from January 2018 to June 2020 with complete follow-up data. There were 35 cases in the simple sclerotherapy group, 104 cases in the simple ligation group, and 27 cases in the ligation combined sclerotherapy group. The results showed that, no serious complications occurred in the 3 groups after surgery. In the simple ligation group and the ligation combined with sclerotherapy group, the incidence of postoperative anal pain [35.6% (37/104) and 33.3% (9/27), respectively,] and anal pendant distension [70.2% (73/104) and 70.4% (19/27), respectively] were higher, but symptoms could be tolerated or relieved after simple treatment. The satisfaction of patients in the 3 groups was all more than 90% before discharge, and the degree of operation acceptance was more than 95%. The effective rate of the 3 groups was above 90.0% at 3 months after surgery, At 12 months after surgery, the effective rate of the simple sclerotherapy group was the lowest [74.3% (26/35)], and the effective rate of the other two groups was still above 85.0%. In conclusion, minimally invasive treatment for internal hemorrhoids under endoscopy is safe and effective with effective improvement of symptoms, high postoperative satisfaction of patients and high degree of acceptance.

Objective:To compare the efficacy and safety of endoscopic sclerotherapy with polycinnamol solution and foam in the treatment of grade II hemorrhagic internal hemorrhoids.Methods:From September 2020 to June 2021, 81 patients with grade II hemorrhagic internal hemorrhoids were collected from the Department of Gastroenterology, the First Affiliated Hospital of University of Science and Technology of China. They were randomly divided into an observation group and a control group. The observation group was injected with polycinnamol solution, and the control group was injected with polycinnamol foam. All of them were treated with endoscopic sclerotherapy. The clinical data of the two groups were compared and analyzed. The operation time, immediate hemostasis rate, incidence of postoperative complications (such as fever, pain, bleeding and Urinary retention), recurrence and rebleeding rate of the two groups were observed, and the efficacy and safety of the two groups in the treatment of grade II hemorrhagic internal hemorrhoids were compared.Results:There was no statistically significant difference in basic data between the two groups of patients (all P>0.05), indicating comparability. The surgical operation time of the observation group patients [(7.40±1.18)min] was shorter than that of the control group [(13.88±0.95)min] ( P<0.05); The injection dose of polycinnamol [(5.79±1.61)ml] in the observation group was higher than that in the control group [(4.38±1.92)ml] ( P<0.05). The immediate postoperative hemostasis rate in the observation group was the same as that in the control group (100%). The incidence of postoperative fever (7.32%), perianal pain (4.88%), bleeding (7.32%), and urinary retention (4.88%) complications in the observation group had no significant difference from that in the control group [postoperative fever (5.00%), anal pain (7.50%), bleeding (7.50%), and urinary retention (2.50%)] (all P>0.05). Two months after surgery, the rebleeding rate in the observation group (4.88%) was not significantly different from that in the control group (7.50%) ( P>0.05), but the rebleeding score in the observation group (1.21±0.63) was lower than that in the control group (2.62±0.71), with a statistically significant difference ( P<0.05). The rebleeding rate (2.44%) and the rebleeding score (2.33±1.51) in the observation group were lower than those in the control group [the rebleeding rate (12.50%) and the rebleeding score (5.54±2.42)] at 12 months after follow-up, and the differences were statistically significant ( P<0.05). Conclusions:Endoscopic sclerotherapy is effective in the treatment of grade II hemorrhagic internal hemorrhoids. There is no significant difference in the immediate and short-term hemostasis rate and the incidence of complications between two different dosage forms of sclerotherapy, namely, polycinnamol solution and foam, but the operation of the solution injection is more time-saving and the long-term recurrence rate is lower, which is worthy of clinical application.

OBJECTIVE:To investigate the effects ofAlisma orientalis polysaccharides on the insulin resistance and lipid metabolism disorder in type 2 diabetes mellitus (T2DM) rats and its possible mechanism.METHODS:T2DM rat model was induced by high fat diet combined with intraperitoneal injection of streptozotocin (30 mg/kg).50 model rats were randomly divided into model group (normal saline),glibenclamide group (positive control,25 mg/kg) and A.orientalis polysaccharide high-dose,medium-dose and low-dose groups (400,200,100 mg/kg),with 10 rats in each group.Other 10 normal rats were included in normal control group (normal saline).All groups were given drugs ig,once a day,for consecutive 6 weeks.After 5 weeks of administration,the glucose tolerance test was conducted.Fasting blood glucose (FBG) levels were measured in rats 0,30,60,90,120 min after intraperitoneal injection of 2 g/kg Glucose solution,respectively.After 6 weeks of administration,serum levels of FINS and FBG were measured,and HOMA-IR and insulin sensitivity index (ISI) were calculated;serum levels of FFA,LDL-C,HDL-C,TG and TC were detected,and the levels of SOD,GSH-Px and MDA in liver tissue were detected.RESULTS:Compared with normal control group,FBG levels of rats in model group at 0-120 min were increased significantly in glucose tolerance test.After 6 weeks of administration,the serum levels of FINS,LDL-C and ISI,and the levels of SOD and GSH-Px in the liver tissue of the model group were decreased,and the differences were statistically significant (P<0.05).Compared with model group,each index of other administration group were improved significantly (P<0.05),except that HOMA-IR,the serum levels of LDL-C and HDL-C were not significantly improved in A.orientalis polysaccharide low-dose group (P>0.05).CONCLUSIONS:A.orientalis polysaccharide can improve insulin resistance and lipid metabolism disorder in T2DM rats through antioxidant effect.

Background:Angiogenesis plays a critical role in tumor growth and metastasis. AGGF1,a new member of angiogenic factors,has been shown to be aberrantly expressed in a variety of malignant tumors. Aims:To investigate the correlation of AGGF1 expression with prognosis of patients with colon cancer. Methods:Eighty colon cancer patients undergoing surgical operation from Jan. 2010 to Dec. 2012 at Xi’an First Hospital were recruited. Immunohistochemistry was performed to evaluate the expression of AGGF1 in cancerous and paired paracancerous tissues. The correlations of AGGF1 expression in cancerous tissue with clinicopathological characteristics and prognosis were analyzed. Cox regression model was used to identify the prognostic factors of colon cancer. Results:AGGF1 expression was significantly higher in colon cancer tissue than in paracancerous tissue (67. 5% vs. 32. 5%,P<0. 05),and was correlated with tumor differentiation,lymph node metastasis,vascular/lymphatic infiltration and TNM stage (P<0. 05). Kaplan-Meier survival analysis showed that high AGGF1 expression was correlated with decrease of survival rate in colon cancer patients (P <0. 05). Univariate and multivariate analyses identified AGGF1 as a prognostic factor of patients with colon cancer (OR=2. 09,95% CI:1. 67-4. 45,P=0. 011;OR =1. 98,95% CI:1. 72-4. 59,P =0. 000 ). Conclusions:Angiogenic factor AGGF1 is up-regulated and correlated with tumor metastasis and unfavorable prognosis in patients with colon cancer. It might be a potential prognostic indicator and deserves further study.

Objective To discuss the clinical application of ultrasound-guided temporary balloon occlusion in performing cesarean section for patients with pernicious placenta previa.Methods At Qujing Municipal First People's Hospital,the Interventional Department and Ultrasonography Department assisted the Obstetrics Department to accomplished cesarean section for 13 patients with pernicious placenta previa.The diagnosis of central placenta previa was confirmed by both color ultrasound and MRI in all the 13 patients.Before cesarean section,the balloons were placed in bilateral common iliac arteries or in abdominal aorta,and the balloons were inflated at the same time when the fetus was delivered so as to temporarily obstruct the target vessels.Based on the hemostatic condition,the balloon was timely withdrawn.Results Among the 13 patients,temporary abdominal aorta occlusion was employed in one and temporary bilateral common iliac artery occlusion in 12.Ultrasound-guided occlusion was successfully accomplished in 12 patients,and the average blocking time was <15 min.The amount of intraoperative blood loss was 800-1500 ml.Conclusion Ultrasound-guided temporary balloon occlusion before cesarean section is safe with reliable effect in treating pernicious placenta previa,it can remarkably reduce the amount of intraoperative blood loos,and it has no X-ray radiation damage,therefore,this technique is worthy of clinical application.

Objective To discuss the clinical efficacy of balloon occlusion technique in treating pernicious placenta previa.Methods Between January 2015 and March 2016,a total of 16 matcrnal patients with pernicious placenta previa,who were admitted to the Interventional Department of Qujing Municipal First People's Hospital,received balloon occlusion management to assist obstetrician in dealing with placenta previa.Central type of placenta previa with highly suspected placenta implantation were diagnosed in 15 patients by color ultrasound or MRI.Balloon catheters were placed in bilateral internal iliac arteries before cesarean section was carried out,and immediately after the fetus was taken out the balloons were inflated to temporary occlude the targeted arteries.Based on the hemostatic status the balloon catheters were timely retrieved.One patient received emergency temporary balloon occlusion of abdominal aorta,and laparotomy revealed that the placenta had penetrated to the subserosa of uterine anterior wall,and total hysterectomy had to be carried out.Results Successful balloon occlusion was achieved in all 16 patients.The mean blocking time of the artery was 15 minutes and the amount of blood loss was 300-1200 ml.Conclusion In treating pernicious placenta previa,balloon occlusion treatment before cesarean section is safe and reliable,it can significantly reduce the amount of blood loss during surgery,make quick and effective hemostasis,provide a clear surgical field for the performance of cesarean section,and,more important,save the maternal life.This effective technique has fully showed the necessity of multidisciplinary collaboration,including obstetrics,interventional radiology,etc.

OBJECTIVE:To study on the phlorotannins in Ecklonia kurome. METHODS:Silica gel column chromatography and Sephadex LH-20 column chromatography were conducted for isolation and identification of phlorotannins,and the compound structures were analyzed and identified based on physicochemical properties and spectral data. RESULTS:Totally 7 phlorotannins were isolated from E. kurome,namely Phloroglucinol(1),Eckol(2),Fucodiphloroethol G(3),Phlorofucofuroeckol A(4),1-(3′, 5′-dihydro-xyphenoxy)-7-(2″,4″,6-trihydro-xyphenoxy)-2,4,9-trihydroxydibenzo-1,4-dioxin(5),Dieckol(6)and 6,6′-bieckol (7). CONCLUSIONS:Compound 5 and 7 are isolated from E. kurome for the first time,and the study has laid a foundation for its quality evaluation.

Objective To investigate the Relationship between the CYP2C19 polymorphism and the efficacy of triple therapy with lansoprazole on Helicobacter pylori infection. Methods 125 elderly patients diagnosed with H.pylori infection were treated with triple therapy. Polymorphism of CYP2C19 was measured by AS-PCR. 105 young patients were selected as control group. The relationship between the polymorphism and eradication rate were analyzed. Results Among the 125 patients,eradication rate of extensive metabolizer group,internal metabolizer group and poor metabolizer groups was 85.29%,76%and 89.39%, respectively. There was no significant difference between groups (P > 0.05). Eradication rate showed no difference between experimental and control group either (P>0.05)(P>0.05). Conclusion The results suggests that the CYP2C19 polymorphism has no correlation with the eradication rate of Helicobacter pylori infection by triple therapy with lansoprazole in elderly patients.

Objective To evaluate surgical approach,safety and efficacy of endoscopic submucosal excavation (ESE) for the treatment of gastric submucosal tumors originated from the muscularis propria layer.Methods Fifty-three patients with submucosal tumors of the stomach diagnosed by gastroscope,were examined using endoscopic ultrasonography (EUS) from February 2012 to April 2013.The patients were completed ESE at general anesthesia.Results The diameter of the tumor was from 5 to 35 mm (median 13.2 mm).The tumors of 48 patients were complete resection,the complete resection rate was 90.6%(48/53).The operation time was from 30 to 150 min (median 45 min).Three patients (5.7%,3/53) had impulsivity hemorrhage during ESE,no patients had unmanageable hemorrhea under the gastroscope.Perforation occurred in 6 patients during ESE,perforation rate was 11.3% (6/53),the perforation was closed by endoclip in 5 patients.Five patients with inability resection and 1 patient with perforation inability closed were treated with surgical operation.None was developed perforation postoperative and hemorrhea.Conclusion ESE is a safety and efficacy method for treating gastric submucosal tumors originating from the muscularis propria layer.

RESUMO

Objective To evaluate the efficacy and safety of endoscopic submucosal excavation in the treatment of suhmucosal tumor.Methods From 2010 to 2013,a total of 108 cases of suhmucosal tumor detected by endoscopic,endoscopic ultrasonography and computed tomography (CT) scan,most of which grew toward the lumen,were treated by endoscopic suhmucosal excavation.After the lesions were completely excavated,pathological examination were performed.All the cases were followed up with endoscopic examination at one,three,six,12 and 24 months after operation.Results Among 108 cases of lesions,27 cases were located in esophagus,eight in cardia,59 in stomach,two in duodenum and 12 cases in rectum.The diameters of the lesions ranged from 0.5 to 5.5 cm (median 2.1 cm).Operation time was between 18 and 240 minutes (median 105 minutes).The endoscopic treatment was converted to laparoscopic operation in three cases because the submucosal tumors of gastric body growing towards outside the lumen,or adhesive to muscular layer tightly and or perforation too big to be sutured.Three cases of submucosal tumor of fundus were failed to excavate,the other 102 cases (94.44%) of lesions were completely removed.Perforation occurred in 19 cases (17.59%).The procedure was converted to laparoscopic operation in one case.Effective endoscopic suture was complered in 18 cases.The volume of haemorrhage during operation was about 0 to 50 mL and no post-operational bleeding was found.Eight cases had cervical subcutaneous emphysema.Nine cases had pneumoperitoneum.Left parotid gland swollen was found in one case.Nine cases were lost to follow up.The left 99 cases were followed up for one to 24 months and no recurrence.Conclusion ndoscopic submucosal excavation safety and effectively achieve once complete removed of the big submucosal tumor which grows towards inside the lumen,and provide complete pathologic data.

Objective To assess the value of ultrasonic probe (USP) in the diagnosis of Submucous eminence of colorectume.Methods Sixty-eight patients with colorectal submucous eminence in 70 areas received USP under colonoscope.The accuracy of diagnosis was evaluated.Results Twenty carcinoid tumor were detected which manifested submucous hypoechoic ; Lipoma 12,located in right half colon which manifested submucous layer,clear boundaries hyperechoic; Cyst 12 manifested single or multi lattices no-echo on submucous which have integrated involucrum.Mesenchymoma or myoma levicellulare 12,most of them located inthe rectum manifested uniform or no-uniform hypoechoic on the muscularis mucosa or below and were difficult to discriminate.Malignant lymphoma,3 manifested muscularis mucosa and submucosa thickening,muscularis propria were seldom affected.Vascular diseases (hemangioma,varicosity) 3,manifested no-echoic on mucosa or submucosa,some medium,high echoic,circular or irregular,and also endometriosis 2,pigment deposition 1,appendix abscess 1,extramural compression 2,ultrasonic test is marched with clinic diagnosis.Accuracy rate is 100%.Conclusion USP can diagnose colorectal submucous eminence with high accuracy,and even provide information about the size,layer of origin,border of the colorectal submucous eminence and can distinguish benign or malignant tumor according to ultrasonic check,at the same time provide differentiation with extramural compressive lesions.

ObjectiveTo explore the viability,security and the curative effect of ultrasonic guided percutaneous portal vein puncture on autologous stem cells transplantation for patients with decompensated liver cirrhosis.MethodsSixty-four patients of decompensated liver cirrhosis with ascites were treated by autologous stem cell transplantation through ultrasonic guided percutaneous portal vein puncture.The clinical symptoms after transplantation,postoperative adverse reaction,the results of liver function tests were observed.Results After stem cells transplantation,patients' liver function got better obviously,laboratory indicators were better,postoperative adverse reaction and complication were less than those of 87patients treated with autologous stem cell transplantation into hepatic artery multilevel branch by pipe(P<0.05).Conclusions Ultrasound guided percutaneous portal vein puncture on autologous stem cells transplantation is the viability,security,less side effect and effective for treatment patients with decompensated liver cirrhosis.It can be used as the clinical treatment methods for patients with middle-late liver cirrhosis.

Stem cells in bone marrow are considered as an ideal sources of seed cells with the advantage of being isolation and easy proliferated in vitro,which is a prerequisite for the autotransplantation. Studies show that the cells can differentiate into neurons. However,this opinion challenges the traditional conception of cell differentiation and therefore the dispute arises. Here we review the results of research on dedifferentiation of bone marrow cells.

Objective:To evaluate the role of the left ventricular ejection fraction (LVEF) in coronary artery bypass grafting(CABG)patients. Methods: From 2004 to 2005,215 patients underwent CABG and there were 36 cases with LVEF0.40(normal LVEF group). Correlative data of the two groups were compared and analyzed statistically. Results: Compared with the normal EF group, the EuroSCORE of the low EF group was much higher (mean 6.4?1.7) and many more patients of the low EF group had concomitant moderate to severe mitral valve insufficiency and aneurysm that needed simultaneous surgical operation(55.6%).Two patients died in hospital(5.5%)in the low EF group whereas five patients in the normal EF group(2.8%, P

RESUMO

<p><b>OBJECTIVE</b>To explore the biological reaction of swine acellular dermal matrix as a filling material for body surface deformity.</p><p><b>METHODS</b>Xenogenic acellular dermal matrix was implanted into the hypoderm of rabbits. The implants were measured and observed with pathological and transmission electron microscopic technique at regular times. The results were compared with implantation of autologous dermis and swine dermis.</p><p><b>RESULTS</b>The implanted xenogenic acellular dermal matrix and autologous dermis had slight inflammatory reaction. At 2 weeks there were fibroblasts and capillaries in the implants. The absorptivity of the xenogenic acellular dermal matrix was lower than the swine dermis. The xenogenic acellular dermal matrix was used in 2 clinical cases and obtained good result.</p><p><b>CONCLUSION</b>The xenogenic acellular dermal matrix is an ideal material with good compatibility feature and low absorptivity.</p>

RESUMO

<p><b>OBJECTIVE</b>To discuss the key role of various mode of circulation assist in the radical operation of retro-peritoneal malignancy with the inferior vena cava thrombotic involvement.</p><p><b>METHODS</b>>From the June 1999 to the March 2001, 6 patients with retro-peritoneal malignancy with thrombosis involving the inferior vena cava, were completely resected with various mode of circulation assist.</p><p><b>RESULTS</b>All the patients operated have good outcomes and follow-up (1 - 20 months) except one death 1 month after operation.</p><p><b>CONCLUSION</b>According to the various types of tumor thrombi, individualized and technically feasible circulation assist mode should be utilized to achieve the satisfactory outcomes.</p>

RESUMO

Object To study the change of serum soluble interleukin 2 receptor(SIL-2R) in syphilitic patients and its clinical significance.Methods The serum SIL-2R level in syphilitic patients of different stages and after therapy were detected using the ELISA technique.Results The serum SIL-2R level in syphiliti patients was higher than that in control group(P0 05).Conclusions SIL-2R level and its variation have some reference value for evaluating the therapeutic effect and judgement of the disease course in syphilitic patients.

RESUMO

Objective: To investigate the roots and canals morphology o f extracted maxillary first premolars. Methods: The root morphol ogy and the root canals of 115 extracted maxillary first premolars were visualiz ed on radiographs taken in the mesio-distal direction, then the teeth were cut transversely and root sections were examined, the root canal systems was analyse d with Vertucci’s classification. Results: 74(64%) teeth were w ith one root, 41(36%) with two roots, all teeth with two roots had two canals a nd each canal with one apical foramen,〔TypeⅠ(1)〕.39% of the single-root teet h demonstrated one canal, whereas 61% of the single-root teeth had two canals, 〔TypeⅡ(2-1), TypeⅣ(2) and TypeⅤ(1-2)〕.Conclusions: The roots of maxillary first premolars possesses a variety of canal system types.
